Btw, the form is the .pdf, not the friendly wording on their site. By signing that form, you are agreeing that not vaccinating puts your child at risk but there is no counter statement about the risks OF vaccinating. Legally, it covers their end but leaves the parent wide open. I would not sign it until I had crossed out that statement.

Yes, the form is at stark odds with their lovely little "respect" blurb.
I asked other mothers who use the clinic, and they said they made no mention face to face of the form.

I sign the form every time, I just cross out and initial the parts I don't agree with.

Yeah, I wouldnt sign that form.
 If they'd accept something else, then id consider it.

I would be tempted to go on pretending I wanted my child vaxed and asking them to sign a form that they are legally liable for the safety of the vaccine and will compensate if there is an adverse reaction...

If they were the most alternate-idea-friendly doc in the area, I would sign the form after I had highly edited it.
